Health sector recruitment in Assam operates on two parallel tracks. NHM Assam drives the larger volume of vacancies, recruiting on a contractual basis for Medical Officers, Staff Nurses, ANMs, Lab Technicians, and programme-specific staff like District Programme Managers and Data Entry Operators. These contractual posts are regularly renewed and sometimes converted to regular positions, making NHM Assam a significant entry point into government health service for medical and paramedical graduates across the state.

The second track is regular recruitment through APSC and the Directorate of Health Services for gazetted Medical Officer cadre posts. These positions carry full state government service status, pension benefits, and career progression through the health service hierarchy. Regular MO posts in Assam are significantly fewer in number than NHM contractual posts, making competition through the APSC channel more intense but the job security considerably better. Many doctors enter service through NHM contracts and simultaneously prepare for the APSC Medical Officer exam.

Government medical colleges are among the most important health sector employers in Assam. Gauhati Medical College & Hospital (GMCH), the state's oldest and largest, and Assam Medical College in Dibrugarh recruit faculty, resident doctors, and technical staff on a recurring basis. The Directorate of Medical Education manages faculty appointments through APSC recommendations, while resident doctor and demonstrator posts are often filled departmentally. FAAMCH in Barpeta and Jorhat Medical College similarly generate nursing, technical, and support staff vacancies through NHM and state government channels.

Paramedical and allied health roles — pharmacists, radiographers, physiotherapists, medical social workers, and hospital administrators — are recruited by NHM Assam as well as by individual medical institutions. The qualification requirements range from diploma-level (for technicians and pharmacists) to postgraduate degrees (for medical social workers and public health specialists).

The ASHA (Accredited Social Health Activist) and ASHA Facilitator programmes generate a distinct category of community health employment at the grassroots level. ASHA Facilitators — who supervise groups of ASHA workers — are recruited district-wise through NHM Assam district offices. These posts require a Class XII pass and preferably a nursing or health-related qualification. While not permanent government jobs, ASHA Facilitator positions provide regular honoraria and exposure to the public health system for women from rural Assam.
